Your First Visit

To schedule an appointment, contact one of our locations. If you cannot attend your scheduled appointment, please notify us at least 24 hours in advance so we can arrange to see patients with urgent needs.

If you are new to our practice, download and fill out the forms on the First Visit Checklist to the right. Please arrive fifteen minutes early so we can process your forms, check insurance and billing information. Please bring your insurance cards and picture identification with you to our office.

Your first examination will be comprehensive, plan on being here for at least one hour. We shall do our best to complete all necessary tests, explain results, confer with you and make recommendations at that time. Before you leave our office every effort will be made to explain the treatment alternatives available for your condition. Please feel free to ask questions at any time.

Our practice accepts most insurance and health plans. The patient is responsible for his/her deductible and any co-payment amount at the time of service. The portion of fees not covered by insurance will be your responsibility. Cash, personal check, credit card or local debit card may be used to make payment in our office. It is our desire to help those who need our services. We are happy to assist you with financial arrangements and appreciate your sharing any suggestions you may have with us. In addition, we will provide significant discounts on self-pay accounts if paid in full at time of service.

Deep Vein Thrombosis

Deep Vein Thrombosis (DVT) occurs when a blood clot forms in one of the deep veins of your body, usually in your legs, but sometimes in your arm.

  • Swelling, usually in one leg
    (or arm)
  • Leg pain or tenderness often described as a cramp or
    Charley horse
  • Reddish or bluish skin discoloration
  • Leg (or arm) warm to touch

Pulmonary Embolism

Clots can break off from a DVT and travel to the lung, causing a pulmonary embolism (PE), which can be fatal.

  • Sudden shortness of breath
  • Chest pain-sharp, stabbing; may get worse with deep breath
  • Rapid heart rate
  • Unexplained cough, sometimes with bloody mucus
